|
Oral: Adults: Initial: 900 mg/day in 3-4 equally divided doses, increase by 300 mg/day at weekly intervals until therapeutic results or toxic symptoms appear Dosing interval in renal impairment: Clcr 10-50 mL/minute: Administer every 8-12 hours Clcr <10 mL/minute: Administer every 12-24 hours |
