Adjunct to anesthesia to induce skeletal muscle relaxation

Stability

Unstable in alkaline solutions; may precipitate if mixed with barbiturates


Usual Dosage

I.V.:

Chronic respiratory paralysis in neonates: Start 0.25-0.5 mg/kg/dose (repeat once if paralysis is not achieved in 3 minutes); maintenance: repeat previous dose as soon as movement is observed. The dose should be titrated to achieve a dosage interval of 3-4 hours, then plateau at 10-20 mg/kg/24 hours

Neuromuscular blockade for surgery: Initial: 0.2-0.4 mg/kg/dose; maintenance: 0.1-0.25 mg/kg/dose every 25-90 minutes

Adults:

Surgery: Initial: 0.2-0.4 mg/kg; supplement dose: 0.5-1 mg; use of anesthetics that potentiate effect of neuromuscular blocking drug requires less metocurine

Electric shock therapy: 1.75-5.5 mg


Administration

IVP over 30-60 seconds


Nursing Implications

Less histaminic release or ganglionic blockade than other curares, otherwise comparable

Injection: 2 mg/mL (20 mL)
